Major Hospital opened the newly designed Sue Ann Wortman Nephrology Center in Summer of 2019 in Shelbyville, IN and houses both MHP’s Nephology Office and the US Renal Dialysis Center. By MHP and Faithful+Gould teaming together for the transition execution, the staff was fully prepared for their first patient day. This presentation will discuss the design of the new building and how Transition Planning was instrumental in supporting the clinical staff in their new space. In addition, we will outline the steps of Transition Planning and will take a tour of the new facility.
